Before You Apply

Application Guidelines

Before submitting your application, please be sure to review the following guidelines carefully.

Ensure that the entire application is completed before submission.

Include a copy of the bill(s) you are requesting assistance with, and make sure your name appears on the bill.

Provide a photocopy of your driver's license.

How to Apply

Download the Financial Assistance form and review the guidelines carefully before filling it out.

Fill out the entire application and gather all required documents including your bill(s) and a copy of your driver's license.

Upload your completed application online or email it to [email protected].

You may also email your completed application to [email protected]

Once your application is submitted, our team will contact you with an update within 5–7 business days regarding the status of your request.
